Create External Reference Types

Integrations that link Rillet records back to their own system attach external_references with a
type slug and a URL. Until now that slug had to be created by hand under Settings → External References
before the first sync. POST /external-reference-types lets the integration create it instead.
Posting the same slug again returns the existing type (200); a new slug returns 201.
